<h3>POSITION SUMMARY</h3><p>TGHC Complex Continuing Care CCC programs provide care for individuals with medically complex conditions and multifaceted chronic diseases requiring integrated and comprehensive low-tolerance long-duration rehabilitation interventions aimed at assisting patients to achieve optimum levels of mobility and physical functioning in order to improve quality of life.</p><h3>Main Responsibilities</h3><ul><li>Assumes responsibility and accountability for the provision of patient care utilizing current physiotherapy best practices and guidelines.</li><li>Develops, implements, and modifies individualized care plans based on physiotherapy assessment findings.</li><li>Evaluates individualized care plans in collaboration with patients, families, and interdisciplinary team members.</li><li>Supervises Physiotherapist Assistants and provides guidance to students and volunteers.</li><li>Acts as a resource and advocate for patients, families, and colleagues.</li><li>Performs any other duties assigned.</li></ul><h3>Qualifications</h3><ul><li>Completion of a Bachelor’s Degree in Physiotherapy or recognized equivalent.</li><li>Registration with the College of Physiotherapists of Ontario in good standing.</li><li>Current Basic Cardiopulmonary Life Saving course certification.</li><li>Demonstrated excellent communication and interpersonal skills.</li><li>Minimum two years of inpatient hospital experience in physiotherapy practice.</li><li>Previous experience working with adults with complex needs.</li></ul><p>Please apply for this exciting career opportunity by emailing your resume and cover letter to <a href='mailto:jobs@torontograce.org'>jobs@torontograce.org</a>, referencing the job posting.</p>
