<h3>Job Requisition: REQ2813</h3><p>Employment Type: Full-time</p><p>Shift: Days</p><p>Hours Per Week: 40/ hours per week</p><p>Hiring Range : $17.17-$22.32</p><p>The actual compensation for this position will be determined based on experience and other factors permitted by law.</p><h3>Responsibilities:</h3><ul><li>Establishes contact and rapport with patients and their families, and provides periodic updates to family members.</li><li>Intermediates between the patient, family members, physicians, community organizations, schools and care providers, as needed to facilitate communication and resolve issues.</li><li>Establishes follow up services, ensuring that all items on the patient care plan have been completed; collaborates with the next level of care including arranging for appointments as indicated.</li><li>Addresses customer service issues with proper follow-up or referral, uses service recovery resources, and communicates issues and resolution to appropriate level.</li></ul><h3>Education:</h3><ul><li>High school diploma/GED with 2 years of experience, or Associate's degree, or <b>Bachelor's degree required.</b></li></ul>
