<h3>Employment Type:</h3><p>Full time</p><h3>Shift:</h3><p>Day Shift</p><h3>Description:</h3><p>Sunnyview Rehabilitation Hospital in Schenectady, NY is seeking a Patient Navigator/Assistant Regional Resource Development Specialist.</p><h3>Responsibilities include:</h3><ul><li>Act as Provider Enrollment Specialist/Provider Recruitment</li><li>Information dissemination and education about waiver services</li><li>Assist with interviewing prospective participants to determine eligibility for Waiver Services</li><li>Track referrals and intakes and manage related databases</li><li>Assist with development of Community Resources and Provider networks</li><li>Assist with review of all participant service plans</li><li>Provide technical assistance to participants, family members and providers</li></ul><h3>Minimum Requirements:</h3><h3>Education:</h3><p>Possess a BS/BA degree or higher in health or human services and 2 years of experience working with individuals with disabilities or seniors which includes case management experience</p><p>OR</p><p>Possess an Associates degree and 4 years of combined professional experience working with individual with disabilities or seniors which includes case management experience</p><h3>Skills/Experience:</h3><ul><li>Must have a working knowledge of community-based services and expertise working with and providing case management services to individuals with disabilities and/or seniors.</li><li>Knowledge of Medicaid, Medicare, third party payers, and 1915 c waivers highly preferred</li><li>Demonstrated expertise working with individuals with disabilities and seniors</li><li>Knowledge, skills, and/or abilities to assess, identify and address gaps in services</li><li>Excellent screening and interviewing skills</li><li>Excellent communication and presentation skills</li><li>Ability to develop and maintain collaborative relationships with regionally based stakeholders, including LDSS, other local government entities, providers, advocacy organizations and others necessary to assure a comprehensive coordinated approach to services for individuals with individuals with disabilities and seniors</li><li>Skill in coordinating activities, evaluating data and establishing priorities</li></ul><p>Pay Range: $21.20-$28.26</p><p>Pay is based on experience, skills, and education. Exempt positions under the Fair Labor Standards Act (FLSA) will be paid within the base salary equivalent of the stated hourly rates.
